Options for Southern Oregon is seeking a Case Manager to join our mobile outreach team in Medford, OR. You will conduct outreach to individuals with behavioral health needs, build trust, and guide them toward treatment options.

The role emphasizes collaboration with community partners, in-office documentation, and driving between sites to support client access to services. Bilingual Spanish skills are a plus and may qualify for language premium.

Qualifications

• Bachelor’s degree in a human services field or three years of relevant experience.
• Interest to register as a CADC-R.
• QMHA status or eligibility with MHACBO registry.

Responsibilities

• Provide outreach and engagement activities to individuals with behavioral health needs.
• Build rapport and facilitate entry into needed treatment.
• Interview and screen clients; help formulate individual treatment plans.
• Assist with accessing local/state/federal resources and entitlements.
