Job summary

Open Sky Community Services in Massachusetts seeks a Clinical Case Manager to provide mobile, in-home, behavioral and community-based support, including for individuals with Autism, under DDS guidance.

Responsibilities include building partnerships, developing individualized treatment plans, coordinating transportation, and participating in treatment planning and crisis support as needed; strong collaboration and documentation are essential.

Qualifications

• Bachelor's degree required.
• Experience supporting people with Autism and developmental disabilities.
• Strong written communication, collaboration, engagement, and organizational skills.
• Ability to provide mobile, in-home, and community-based services and transportation support.

Responsibilities

• Establish respectful partnerships with people served, families, staff teams, and other stakeholders.
• Provide mobile support to individuals with Autism referred by DDS Area Offices.
• Provide in-home, behavioral, clinical, and community-based support to individuals.
• Assist people served to access transportation and provide transportation as needed.
• Operate within Positive Behavioral Supports and assist PBS training for staff, families, and stakeholders.
• Train participants and collateral supports in identified interventions and strategies.
• Consult with internal and external teams regarding clinical needs.
• Prepare individualized treatment plans with assigned individuals and families.
• Participate in treatment planning to develop goals and objectives.
• Complete assessments, treatment plans, and documentation on time.
• Attend meetings, trainings, and supervision as assigned.
• Participate with the treatment team in completing assessments and Individual Support Plans.
• Apply motivational interviewing and engagement techniques to build relationships and goals.
• Facilitate groups and organize events based on participant needs.
• Act as liaison with treatment teams, including DDS and allied health providers.
• Maintain current files and databases for individuals served.
• Participate in crisis support as needed.
• Perform other duties as assigned.
