Open Sky Community Services is seeking a Clinical Case Manager to provide mobile, in-home, and community-based support for young adults and adults with Autism, intellectual and developmental disabilities, and mental health needs.
The role emphasizes problem-solving, social connection, and executive functioning, using motivational interviewing and a person-centered, trauma-informed approach to promote housing stability, community engagement, and wellness.
Clinical Case Management is a more intensive service than traditional case management. Individuals referred to the program may require a higher degree of support, most often provided in their homes or in the community. The Clinical Case Manager supports young adults and adults with Autism, intellectual and developmental disabilities, and mental health needs. The role teaches and reinforces skills in problem-solving, social connection, executive functioning, and other areas relevant to each person’s goals. Services promote stabilization in housing, engagement, community connection, relationships, mental health, and progress toward future goals. The Clinical Case Manager brings curiosity, uses motivational interviewing and engagement techniques, remains open to new ideas, and collaborates with Open Sky colleagues, families, funders, providers, and other community partners. All Open Sky staff promote community membership by connecting or reconnecting people served to meaningful opportunities and mutually supportive relationships while promoting wellness. The Clinical Case Manager delivers services through a person-centered, trauma-informed lens and demonstrates a steadfast commitment to diversity, equity, inclusion, and belonging.
Pay Range: $31.52 - $31.52
