Life Center of Galax seeks a Mobile Assessor to expand referral networks and coordinate admissions for clients with substance use recovery goals.

You will collaborate with hospital discharge planners, social workers and physicians, manage leads, educate stakeholders and travel within the community to develop sources. A high school diploma with 2+ years in sales/marketing is required; a marketing degree is preferred.

Qualifications

• Two or more years of experience in sales and/or marketing.
• Bachelor's degree in Marketing or related health or social services field preferred.
• High school diploma or equivalent required.

Responsabilités

• Collaborate closely with hospital discharge planners, social workers, case managers, physicians and management to generate referral sources.
• Develop strong relationships with key referral sources.
• Manage leads from referral sources and ensure efficient admissions process for clients.
• Travel frequently within community to schedule meetings and develop contacts/referral sources.
• Educate patients, families, community and other referral sources on programs, services and amenities provided within facility.
• Coordinate the completion of admission process and ensure appropriate resources are obtained.
• Utilize analytic tools, such as Salesforce, to effectively plan and evaluate activity.
• Complete on-site assessments with patients to coordinate admissions.
• Assist with transportation, as needed.

Pay Range

$22/hour - $28.08/hour

PURPOSE STATEMENT

Expand lines of service as defined by management by developing and retaining referrals from physicians and other key non-physician referral sources

Responsibilities

ESSENTIAL FUNCTIONS

• Collaborate closely with hospital discharge planners, social workers, case managers, physicians and management, as well as provide education staff to generate referral sources.
• Develop strong relationships with key referral sources.
• Manage leads from referral sources and ensure efficient admissions process for clients.
• Increase the number of referral sources within a facility such as with physician, social workers and discharge planners.
• Coordinate with clinical assessment team on referrals for admission based on the services provided by the facility.
• Educate patients, families, community and other referral sources on programs, services and amenities provided within facility.
• Coordinate the completion of admission process and ensure appropriate resources are obtained.
• Travel frequently within community, scheduling meetings and developing contacts/referral sources.
• Maintain existing referral relationships.
• Utilize analytic tools, such as Salesforce, to effectively plan and evaluate activity to optimize results.
• Complete on-site assessments with patients to coordinate admissions.
• Assist with transportation, as needed.

OTHER FUNCTIONS

• Perform other functions and tasks as assigned.

Qualifications

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS
