Job summary

Brigade Health (Access Healthcare Associates) is seeking a compassionate NP or PA to join our Mobile Primary Care Team serving older adults in homes and senior living communities. You’ll deliver high-quality, individualized care with autonomy and purpose, supported by a multidisciplinary network.

The ideal candidate holds an active NP/PA license, 1–2 years in geriatrics or primary care, and a patient-centered approach with strong clinical judgment.

Qualifications

• Active, unencumbered NP or PA license in the practicing state.
• 1–2 years of clinical experience in geriatrics or primary care is preferred.
• Compassionate, patient-centered approach and strong clinical judgment.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver personalized, high-quality care to older adults in homes and senior living settings.
• Conduct comprehensive assessments and develop individualized care plans.
• Manage chronic conditions with skill and empathy.
• Collaborate across clinical disciplines for whole-person care.
• Build trusted relationships with residents, caregivers, and partners.
• Maintain thorough documentation and consistent communication.
