Brigade Health seeks a compassionate Nurse Practitioner or Physician Assistant to join our Mobile Primary Care Team, delivering in-home and senior-living-based medical services. You will conduct assessments, develop care plans, and manage chronic conditions with a focus on geriatrics and patient-centered care.

The role emphasizes autonomy within a collaborative, supportive environment, with opportunities to grow within a rapidly expanding organization and serve older adults in diverse settings.

Competenze

• Active NP or PA license in the state of practice.
• Minimum 1–2 years of clinical experience in geriatrics or primary care.
• Compassionate, patient-centered approach with strong clinical judgment.
• Valid driver's license and reliable transportation.
