Job summary

L.A. Care Health Plan is looking for a Managed Long Term Services and Supports Coordinator II in Los Angeles, CA. This full-time role focuses on outreach and support for members accessing services. The ideal candidate will have an Associate’s Degree and at least one year of related experience in managed care or as a Medical Assistant.

Responsibilities include collaborating with various departments, providing information about MLTSS services, and coordinating care. Strong proficiency in Microsoft Word and Excel, along with excellent organizational skills, is required.

Qualifications

• At least 1 year of related experience in managed care or as a Medical Assistant is preferred.
• Bilingual in threshold languages of L.A. Care Health Plan is desirable.

Responsibilities

• Outreach to members based on HRA data assessing needs or barriers in care.
• Implement new hire orientation for various departments as needed.
• Handle Long Term Care Institutional Member reassignment and discharge summaries.
• Act as subject matter expert in the Model of Care.
• Troubleshoot iPPG reassignment issues and perform other related duties.
