Qualifications

Responsibilities

• Counsel assigned consumers and manage caseloads in the Mission Services program.
• Coordinate services, referrals, and crisis intervention as needed.
• Document services and write progress and discharge summaries.
• Monitor progress and adjust Mission Services plans over time.
• Prepare discharge summaries for referral agencies and conduct exit interviews.
• Collaborate with supervisors and other agencies to support goals.

Essential duties and responsibilities:

• Responsible for caseload assigned of consumers. Counsels consumers, moves them into work stations, follows progress, reports results in case conferences, writes reports for referring agency, and maintains the files of the consumers assigned.
• Ensure assigned consumers on the caseload receive the needed services in order to progress in their Mission Services plans such as counseling, referrals to other agencies, and crisis intervention.
• Collaborate with supervisors and work with consumers to increase productivity and meet the goals of the Mission Services plan.
• Document in consumer files the services rendered.
• Write Mission Services plan for each consumer as they progress through the program. Monitor and evaluate progress and modify as needed.
• Write discharge summary for referral agency of each consumer leaving program. Conduct exit interview and collect I.D. badge from each.

• Education, Training and Experience:
  • Associate's degree required; Bachelor'sdegree preferred.
  • Special training helpful in human relations, positive confrontation techniques, behavioral science, marketing and communications.
• • Previous experience in a rehabilitation setting.

  • Knowledge of Mid-Michigan agencies.
