Job summary

Wellpath is seeking a Licensed Practical Nurse (LPN) in Louisiana to support patient care under RN supervision. You will assess patients, administer medications, and assist in procedures, contributing to individualized treatment plans with quality documentation.

The role requires an active LPN/LVN license in the state, CPR certification, and a minimum of one year clinical experience. This is a full-time on-site position offering comprehensive benefits and ongoing professional development.

Qualifications

Graduate from an accredited school of nursing.
Minimum 1 year clinic experience preferred.
Current licensure as Licensed Practical/Licensed Vocational Nurse in the state of employment.

Responsibilities

Under RN supervision, assess patients’ physical, psychological, and social dimensions to inform treatment planning.
Administer medications, including IV therapy if certified, following protocols.
Assist in procedures and document nursing encounters using SOAP charting for clear communication.
Attend mandatory meetings and training to maintain high standards of care.
