Universal Health Services, through Lincoln Trail Behavioral Health System, is seeking a Military Care Coordinator to coordinate admissions and after-care for service members, veterans, and first responders within a multidisciplinary team.

Based in Radcliff, KY, the role partners with clinicians, patients, and referral sources to ensure smooth transitions from admission to discharge and connect patients with appropriate continuing care resources.

Qualifications

• Bachelor's degree in Human Services field is required.
• Experience in healthcare setting is preferred.
• Two years working in a human service organization is preferred.

Lincoln Trail Behavioral Health System is a 140-bed hospital offering inpatient services for adolescents, adults, military, veterans and first responders. Lincoln Trail Behavioral Health System has a spacious cafeteria, gymnasium, art/activity room, group rooms, classrooms and other areas to create a pleasant atmosphere for patients and their families. Lincoln Trail Behavioral Health System's quality services are provided by psychiatrists, licensed therapists, certified alcohol and drug counselors, activity therapists and teachers. Also, care is given by registered nurses 24 hours a day. Since 1986, Lincoln Trail Behavioral Health System has been a proven psychiatric care provider in mental health and substance use.

The Military Care Coordinator is a key member of the Lincoln Trail Behavioral Health System social services team. The Military Care Coordinator is responsible for coordinating admissions and the after-care needs of each service member, veteran, or first responder based on recommendations of the clinicians, base commands, and patient goals.

Job Duties/Responsibilities:

• Coordinates care of patients from admission to discharge
• Collaborates with clinician, patients and referral sources
• Links patients to appropriate continuing care resources
