Northeastern Vermont Regional Hospital (NVRH) in St. Johnsbury, Vermont, seeks an EMS Mobile Integrated Health RN Program Coordinator to lead clinical and operational aspects of the Northeast Kingdom MIH program.

You will oversee patient referrals, coordinate with EMS field teams, medical directors, and community partners to connect patients with the right care at the right time, driving population health improvements.

Formación

• Current, unrestricted Vermont or Multistate RN license.
• Minimum three years of clinical experience in emergency nursing, EMS, critical care, community-based care, or a comparable clinical setting requiring independent clinical judgment.
• Experience with patient care coordination and multidisciplinary collaboration.
• Strong clinical assessment, communication, organizational, and problem-solving skills.
• Ability to manage multiple priorities independently and maintain accurate documentation.
• Proficiency with electronic health records and documentation systems.
• Valid driver's license.

Responsabilidades

• Coordinate patient enrollment, scheduling, care planning, follow-up activities, and ongoing program participation while maintaining the MIH patient census and monitoring progress toward care goals.
• Serve as the primary liaison between patients, EMS agencies, healthcare providers, home health organizations, and community partners to ensure seamless communication, coordination, and continuity of care.
• Support EMS field teams through clinical coordination, patient information review, identification of escalation needs, and coordination of appropriate follow-up for clinical and social determinants of health needs.
• Co-lead quality improvement activities, monitor program outcomes and documentation quality, support grant reporting requirements, and participate in ongoing program development and evaluation efforts.
