Job summary

UVM Health Network - Central Vermont Medical Center is seeking a Community Paramedic to join our MIH program. You will provide patient-centered care in homes and community settings across central and northeast Vermont, perform assessments, education, and care coordination, and support transition from hospital to home.

Responsibilities include chronic disease management, medication reconciliation, post-hospital follow-up, and outreach to rural communities.

Qualifications

• High School Diploma or GED required; degrees preferred.
• Active Vermont EMS Paramedic Certification required.
• NREMT-P certification required.
• CP-C certification preferred after 12–24 months.
• 3+ years of direct patient care in 911, critical care, or community paramedicine.

Responsibilities

• Perform assessments and patient education in homes and community settings.
• Assist with chronic disease management and medication reconciliation.
• Provide post-hospital follow-up and care coordination.
• Support outreach visits to rural communities and navigation of services.
• Collaborate with a broader care team including behavioral health and social services.
