Job summary

The University of Vermont Health Network is seeking a Community Paramedic for the UVMH Mobile Integrated Health program in Berlin, VT. This benefits-eligible, grant-funded position supports home visits, community settings, and non-traditional care environments, with a 20-hour per week schedule and day/evening shifts.

Qualifications

• High school diploma or GED required.
• Paramedic certification (Vermont) required.
• NREMT-P certification required.
• 3+ years of direct patient care in 911, critical care, or community paramedicine.
• CP-C certification preferred after 12–24 months of hire.

Responsibilities

• Provide patient-centered care in homes, community settings, and non-traditional environments.
• Conduct assessments, education, chronic disease management, and medication reconciliation.
• Post-hospital follow-up and care coordination with providers, behavioral health, case managers, and partners.
• Support program goals to improve access and reduce avoidable ER visits and readmissions.
• Perform outreach to rural communities and support home-visitation service models.
