Job summary

The University of Vermont Health Network is seeking a Community Paramedic to join an interdisciplinary mobile integrated health program serving central and northeast Vermont. You will conduct patient assessments, provide education, support chronic disease management, and coordinate care with a broad care team.

This benefits-eligible, grant-funded role emphasizes outreach to rural communities, post-hospital follow-up, and reducing avoidable ED visits through home visits and care coordination.

Qualifications

• High School Diploma or Equivalency (GED) required.
• Associate’s or Bachelor’s Degree preferred.
• Vermont Office of Emergency Medical Services Paramedic Certification (required).
• National Registry Emergency Medical Technician Paramedic (NREMT-P) (required).
• New York Emergency Medical Services Paramedic Certification (preferred).
• CP-C (Community Paramedic Certification) preferred after 12–24 months of hire.
• 3 or more years of direct patient care experience in 911, critical care, or community paramedicine.

Responsibilities

• Conduct assessments and provide patient education in homes, community settings, and other non-traditional environments.
• Assist with chronic disease management, medication reconciliation, and post-hospital follow-up.
• Coordinate care with physicians, behavioral health specialists, case managers, and community partners.
• Perform outreach visits to rural communities and support home-visitation service models.
