Job summary

Copley Hospital in Morrisville/Stowe VT is seeking a Mobile Integrated Health (MIH) Paramedic to provide in-home assessments, follow-up, education, and care coordination for patients in the community.

The MIH Paramedic collaborates with the care team to monitor progress, identify barriers, and connect patients with essential services, aiming to prevent avoidable ER visits and hospital readmissions. Travel to patient homes is required, and this is a full-time role with varied shifts.

Qualifications

• Two or more years of paramedic experience.
• Current VT Paramedic licensure or ability to obtain before providing care.
• Current Basic Life Support with ACLS and other program certifications.

Responsibilities

• Perform home visits and follow-up contacts post-discharge or after ED visits.
• Provide education and support to reinforce discharge instructions and care plans.
• Coordinate care and escalate as needed with the care team.
• Document all encounters in the electronic record within required timeframes.
• Maintain equipment and travel readiness; ensure safety in field visits.
