Job summary

Qualifications

• High School Diploma or GED required.
• Associate’s or Bachelor’s Degree preferred.
• Vermont OEMS Paramedic Certification required.
• NREMT-P required.
• New York EMS Paramedic Certification preferred.
• CP-C certification preferred after 12–24 months of hire.
• 3+ years of direct patient care experience in 911, critical care, or community paramedicine.

Responsibilities

• Conduct assessments and provide patient-centered care in homes, community settings, and other non-traditional environments.
• Assist with chronic disease management and medication reconciliation.
• Provide post-hospital follow-up and care coordination.
• Support patients in navigating healthcare and social resources; coordinate with providers and case managers.
• Perform outreach visits to rural communities and support home-visitation models.
