Randstad USA in Wallingford, CT is seeking a qualified Mid-Shift Pharmacist for a contract role. The ideal candidate has LTC pharmacy experience, a PharmD, and CT license, and will cover 11:00 AM – 7:00 PM.

You will review, dispense, counsel, monitor adherence, and collaborate with providers to optimize patient care while ensuring compliance with state and federal laws. This role emphasizes patient safety, medication management, and education.

Formación

• PharmD degree required and CT license in good standing.
• LTC pharmacy experience required.
• Strong knowledge of medication management and drug interactions.
• Excellent communication and organizational skills.
• Ability to work independently and in a fast-paced environment.

Responsabilidades

• Review, verify, prepare, and dispense prescription medications in accordance with applicable laws and regulations.
• Monitor patient medication compliance and identify potential misuse or overuse.
• Counsel patients and healthcare professionals regarding medication use, dosage, storage, side effects, and potential drug interactions.
• Provide consultation on prescription and over-the-counter medications.
• Identify drug-related problems and communicate recommendations to physicians and other healthcare providers.
• Collaborate with physicians, nurses, pharmacists, pharmacy interns, and other healthcare professionals to optimize patient care.
• Provide patient education and support services, including blood pressure monitoring, cholesterol management, and smoking cessation.
• Educate patients on the proper use and handling of medical devices, including blood pressure monitors and glucose meters.
• Compound medications for internal and external use.
• Prepare sterile solutions in accordance with established procedures and safety standards.
• Supervise and support pharmacy technicians, clerks, and other pharmacy staff.
• Maintain appropriate inventory levels of medications and medical supplies.
• Ensure compliance with all state and federal pharmacy laws, regulations, and professional standards.
