Job summary

PharMerica in Wallingford, CT is hiring a full-time Pharmacist to join our closed-door, non-retail team serving long-term care and senior living clients.

You will evaluate medication orders, provide clinical recommendations, maintain patient profiles, and collaborate with physicians and nurses to ensure safe, effective therapy. The role requires licensure in CT and at least one year in long-term care; schedule is Mon-Fri with rotating weekends, benefits included.

Qualifications

• Bachelor's Degree in Pharmacy; Pharm D preferred.
• One year experience in long-term care desired.
• Current pharmacist license in the state of practice (CT).
• Knowledge of CMS, DEA, FDA guidelines and drug therapy safety.
• Proficient with EMR and pharmacy systems.
• Strong clinical judgement and communication skills.

Responsibilities

• Evaluate physician orders for drug appropriateness, dose, and interactions.
• Provide clinical recommendations to physicians and staff.
• Maintain patient/resident profiles with medical history and meds.
• Ensure lab monitoring for high-risk medications is ordered and reviewed.
• Collaborate with healthcare professionals to support therapy.
• Document interventions in the medical record.
• Ensure compliance with state and federal pharmacy regulations.
• Educate staff on safe medication handling.
