PharMerica in Wallingford, CT is seeking a PRN Pharmacist to join our closed-door pharmacy serving long-term care and senior living clients. This non-retail role emphasizes clinical collaboration and patient-focused care.

You will evaluate orders, provide recommendations, maintain patient profiles, ensure monitoring tests, and support the healthcare team. Scheduling includes weekdays and rotating shifts; benefits are offered.

Qualifications

• Bachelor’s degree in Pharmacy; PharmD preferred.
• One year experience in long-term care desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines, medication safety, and CMS/DEA/FDA standards.
• Proficiency with EMR and pharmacy information systems.
• Strong clinical judgement and effective communication.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and staff on therapy changes.
• Maintain up-to-date patient profiles, including medical history, labs, and meds.
• Ensure monitoring tests for high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses, and other professionals for clinical support.
• Document communications and interventions in the medical record.
• Ensure compliant handling, dispensing, and oversight of medications.
