RHA Health Services, LLC is seeking an IDD Care Manager for Wake & surrounding counties. This community-based hybrid role requires travel and offers flexible schedules.

The ideal candidate will have a human service degree and experience with the MH/SU/IDD population. Responsibilities include care management assessments, monitoring care plans, and collaboration with community organizations.

Qualifications

• Master’s degree in a human service field with at least one year of experience.
• Bachelor’s degree in a human service field with two years of experience.
• Bachelor’s degree in a non-human service field with four years of experience.

Responsibilities

• Foster and facilitate the care management process.
• Conduct Care Management Comprehensive Assessments.
• Monitor ongoing care management plan implementation.
• Maintain a person-centered philosophy.
• Establish and maintain strong external relationships.
