Job summary

RHA Health Services, LLC seeks a dedicated Care Manager to deliver whole-person, integrated care across sites, homes, and communities. You will support individuals with mental health, substance use, and developmental disabilities, coordinating care through interdisciplinary teams and community partners.

The role emphasizes tailored assessment, plan development, and ongoing monitoring to improve health outcomes while complying with policies and delivering high-quality services.

Qualifications

• Care managers must meet at least one qualification per 10A NCAC 27 .0104.
• Experience in care coordination, care management, or case management.

Responsibilities

• Fosters and facilitates the care management process and conducts comprehensive assessments.
• Monitors care plan implementation and updates plans annually or with significant changes.
• Collaborates with Integrated Care Team and community providers to ensure success of care management.
• Maintains a person-centered, data-driven approach to plan and document services.
