Job summary

ViaQuest, Inc. is seeking a Mental Health Therapist (LCSW or LMHC) for a full-time role in an Indiana long-term care setting. You will assess, treat, and monitor adults, delivering visits and updating plans to ensure the best outcomes.

This position offers comprehensive benefits, training, and opportunities for ongoing education, with travel within the service area and no weekend shifts.

Qualifications

• Master’s degree in social work or professional counseling required.
• LCSW or LMHC license required or equivalent.
• 2 years’ therapy/counseling experience preferred.
• Valid driver’s license and reliable transportation.
• Willingness to travel throughout the service area.

Responsibilities

• Perform comprehensive diagnostic assessments.
• Provide patient visits as scheduled.
• Ensure treatment plans are up to date for each client.
• Complete comprehensive progress notes.
• Communicate and follow up regarding client status.
• Collaborate with case management and psychiatric services.

Our clients are supported by a team of Certified Nurse Practitioners and Mental Health Counselors and Social Workers to ensure the best outcomes for those we serve.

Responsibilities may include:

• Performing comprehensive diagnostic assessments.
• Providing patient visits as scheduled.
• Ensuring treatment plans are up to date for each client.
• Completing comprehensive and succinct progress notes.
• Communicating and following up as needed regarding the client's status.
• Working collaboratively with those providing case management and/or psychiatric services.

Requirements for this position include:

• Master’s degree in social work or professional counseling
• Social Work/Professional Counseling License or equivalent: LCSW or LMHC
• 2 years’ experience in therapy and counseling is preferred.
• Valid driver's license, car insurance and reliable transportation.
• Willingness to travel throughout assigned service area.
