Qualifications

Responsibilities

• Applies root cause analysis to readmissions and collaborates with the health care team to resolve issues.
• Communicates and collaborates with the multidisciplinary team to advise on the plan and patient response.
• Develops and implements plan of care based on assessment including counseling or crisis intervention; links to community resources.
• Promotes the profession of social work through education and teaching rounds; provides on-call services per policy.
• Responds to referrals and assesses psychosocial and complex discharge needs using data from medical records and team.
