Job summary

Horizon House, Inc. in Claymont, Delaware seeks an Associate Mental Health Specialist to join the ACT multidisciplinary team. This role provides rehabilitation, support services, crisis intervention, and coordination with families and community agencies to ensure integrated treatment plans.

Responsibilities include developing treatment goals with participants, coordinating with employment services, and offering on-call crisis intervention under supervision. Valid driver’s license required.

Qualifications

• Bachelor's degree in social work, psychology, rehabilitation or mental health counseling, or a related human services field.
• Knowledge of mental health and substance abuse systems and supports.
• Ability to work independently and as part of a focused team.
• Good communication skills both oral and written.
• Possess a valid driver’s license with an acceptable driving record.

Responsibilities

• Provide rehabilitation and support functions as part of the ACT multidisciplinary team.
• Provide crisis intervention under clinical supervision.
• Act as liaison with community agencies and families to coordinate treatment.
• Provide case management for assigned participants, develop and revise treatment goals.
• Provide on-call crisis intervention during nighttime hours.
• Coordinate with state vocational rehabilitation and other employment services.
