Job summary

Devereux is seeking a Blended Service Coordinator to provide professional assessment, service planning, linkage, referral, and advocacy for individuals in the Greater Pittsburgh area. You will work on a treatment/service team to promote mental health recovery, with monthly contact expectations and a 37.5-hour training annual requirement.

Qualifications

• Bachelor’s degree with major course work (min. 12 credit hours) in social/behavioral field or nursing.
• Minimum of 1 year of experience in human services, community based preferred.
• Clearances (Act 33/34) and FBI Clearance required.

Responsibilities

• Advocate for individuals to ensure access to natural, community and specialized services.
• Maintain timely records of service coordination activities and prepare for audits.
• Coordinate resources with families to meet individual needs and link to services.
• Provide on-site support for inpatient mental health commitments when alternatives to hospitalization have been considered first.
• Provide culturally competent services respecting diverse backgrounds.

Education

Bachelor’s Degree in Social Work, Psychology, Criminal Justice, Pastoral Counseling, Counseling Psychology, Rehabilitation Counseling, Sociology, Education with a certificate in Special Education or Nursing with a Registered Nurse License
