Job summary

Devereux is seeking a Blended Service Coordinator to support clients in the Greater Pittsburgh Area. You will coordinate mental health services, advocate for individuals, and maintain detailed documentation to support recovery-oriented care.

Qualifications include a Bachelor’s degree in relevant fields, 1+ year of human services experience, and state clearances. The role emphasizes culturally competent service delivery and regular engagement with clients and families.

Qualifications

• Bachelor’s Degree from an accredited college or university in listed fields.
• Minimum of 1 year of experience in human services; community-based preferred.
• Act 33/34 Clearances and FBI Clearance.
• Valid PA driver’s license with 3 years of driving history and MVR requirement.

Responsibilities

• Advocate for individuals to access natural supports and community services.
• Maintain accurate records of service coordination within 24 hours and ensure regulatory compliance.
• Coordinate resources to meet individual needs through linkage and referral.
• Provide on-site support for in-patient MH commitments after considering alternatives to hospitalization.
• Deliver culturally competent services respecting diverse backgrounds.
• Attend required trainings (37.5 hours annually) to stay current on best practices.
• Maintain monthly contact and provide full service coordination for caseload.

Education

Bachelor’s Degree with major course work in Social Work, Psychology, Criminal Justice, Pastoral Counseling, Counseling Psychology, Rehabilitation Counseling, Sociology, Education with a certificate in Special Education or Nursing with a Registered Nurse License
