Job summary

Devereux Advanced Behavioral Health is seeking a Blended Service Coordinator to assess, plan, coordinate and advocate for mental health services for a designated caseload in the Greater Pittsburgh Area. You will join a treatment team, promote recovery, and support individuals through community resources and natural supports.

The role emphasizes advocacy, timely documentation, and ensuring access to appropriate services, with on‑call weekend coverage and a 9–5 weekday schedule.

Qualifications

• Bachelor’s Degree in relevant field as listed (Social Work, Psychology, Criminal Justice, Pastoral Counseling, Counseling Psychology, Rehabilitation Counseling, Sociology, Education with Special Education, or Nursing with RN license).
• Minimum 1 year of experience in human services, community based preferred, and working knowledge of human services paperwork and regulation guidelines.
• Act 33/34 (PA State Criminal and Child Abuse) Clearances and FBI Clearance.
• Valid PA driver’s license and driving history of three (3) years with MVR required.

Responsibilities

• Advocate for and with the individual being served to ensure responsiveness from natural, community, geriatric and specialized services/supports.
• Maintain an accurate and timely record of Service Coordination activity by completing service documents within 24 hours.
• Ensure needs are met through natural supports, community and specialized services; assist individual and family to identify, link, access and coordinate resources.
• Provide on-site support for all in-patient MH commitments by considering alternatives to hospitalization first and advocating for admission.
• Provide culturally competent services respecting diverse backgrounds.
• Attend required trainings (37.5 hours annually).
• Ensure monthly contact at a minimum and manage a designated caseload.

Education

Bachelor’s Degree with major course work (min. 12 credit hours) in Social Work, Psychology, Criminal Justice, Pastoral Counseling, Counseling Psychology, Rehabilitation Counseling, Sociology, Education with a certificate in Special Education or Nursing with a Registered Nurse License. All degrees must come from an accredited college or university.
