Job summary

Crestwood Eureka is seeking a Service Coordinator (Mental Health Case Manager) to assess clients, identify treatment needs, and coordinate interdisciplinary care plans within a supportive behavioral health environment.

The role requires at least two years of mental health experience, a relevant bachelor’s degree (or substitution as stated), and knowledge of DSM-5 and regulatory standards. This is a full-time on-call position with competitive benefits in Eureka, California.

Qualifications

• Two (2) years of full-time experience or part-time equivalent in a mental health setting.
• Bachelor’s degree with a major in psychology, social work, or behavioral sciences may be substituted for the two (2) years of full-time work experience or its part-time equivalent.
• Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
• Familiarity with regulatory standards.

Responsibilities

• The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments.
• The Service Coordinator coordinates the interdisciplinary team meetings and discharge planning among the treatment team.
• In this capacity, coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.
