Job summary

Crestwood Behavioral Health is seeking a Service Coordinator (Mental Health Case Manager) in Angwin, CA. You will assess clients, plan care, and coordinate interdisciplinary teams to support recovery goals.

Qualifications

• Two years of full-time experience or part-time equivalent in a mental health setting.
• Bachelor’s degree in psychology, social work, or behavioral sciences may substitute for two years of experience.
• Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
• Familiarity with regulatory standards.

Responsibilities

• Assess clients to identify individual problem areas necessitating treatment and complete assessments.
• Coordinate interdisciplinary team meetings and discharge planning among the treatment team.
• Coordinate overall treatment plan implementation with nursing staff to ensure continuity of care.
