Job summary

Mental Health Resource Center in Florida provides FACT services to adults with severe mental illness through a multidisciplinary team, aiming to reduce hospitalization, increase community tenure, and enhance independence, autonomy, and quality of life.

The FACT team delivers advocacy, crisis intervention, assessments, treatment planning, case management, medication support, and care coordination, with community-based services and some office time.

Qualifications

• Master’s degree in Human Services or related field required.
• Experience working with adults with severe and persistent mental illness required.
• Proficiency in RBHS/MHRC EHR and patient information systems within 3 months.
• Proficiency in Microsoft Office and general internet use.
• Valid Florida driver’s license and acceptable driving record per MHRC requirements.
• Ability to work in homes, offices, hospitals, nursing homes, and secure units.

Responsibilities

• Provide treatment, rehabilitation, and support per FACT model.
• Maintain confidential clinical records accurately.
• Conduct mental health status assessments.
• Coordinate referrals to community providers and support services.
• Assist with daily living activities, transportation, and social skills.
• Educate on mental health symptoms, medications, and recovery.
• Conduct individual and group therapy sessions.
• Serve as primary case manager for assigned caseload.
