Radias Health is seeking a Clinical Lead Mental Health Practitioner to oversee a four-resident group home and provide supervision and clinical direction to staff in the Residential Support Services program. Responsibilities include coordinating client care, updating treatment plans, and collaborating with the treatment team, hospital staff, and community professionals.

The role requires an associate or bachelor’s degree in human services and a valid driver’s license.

Qualifikationen

• Requires an associate or bachelor’s degree in human services with relevant full-time experience.
• Must have car, auto insurance and valid driver’s license.
• Experience coordinating client care and treatment plans is preferred.

Aufgaben

• Coordinate client care and quarterly case conferences with the treatment team and family.
• Develop and review service and treatment plans in collaboration with supervisors.
• Schedule medical appointments, referrals, and transportation for clients.
• Assist with crisis de-escalation and safety planning as needed.
• Lead team meetings and ensure proper documentation and reporting.
