Job summary

Pathways Forward is seeking a Forensic Intake/Administrative Case Manager to provide assessment, case management, service coordination, advocacy, and support to individuals involved with the criminal justice system.

This role works closely with consumers, families, mental health providers, courts, correctional facilities, and community agencies to promote continuity of care and access to appropriate services.

Qualifications

  • Bachelor's degree in sociology, social welfare, psychology or related field.
  • Experience with criminal justice or forensic mental health settings is a plus.
  • Two years of experience in public or private human services OR five years of mental health direct-care experience.

Responsibilities

  • Conduct intake interviews and assessments to determine eligibility and needs.
  • Maintain a caseload for MH Jail Diversion and others in the justice system.
  • Coordinate mental health services with courts, facilities, providers, and agencies.
  • Attend court hearings and advocate for consumers as appropriate.
  • Assess emergencies and make referrals, including admissions when necessary.
  • Coordinate access to mental health and case management services.
  • Complete assessments, referrals, releases of information and documentation.
  • Monitor participation and progress via calls and in-person contacts.
  • Facilitate ISPT meetings and treatment/discharge planning.
  • Develop supportive relationships with consumers and partners.
  • Advocate for resiliency and recovery-oriented goals.
  • Maintain accurate records and participate in trainings and meetings.
  • Support agency mission and ethics; undertake special projects.

Pathways Forward is seeking a Forensic Intake/Administrative Case manager to provide assessment, case management, service coordination, advocacy, and support to individuals involved with the criminal justice system.

This position works closely with consumers, families, mental health providers, courts, correctional facilities, community agencies, and other stakeholders to promote continuity of care and assist individuals in accessing appropriate mental health and community-based services.

Key Responsibilities:

  • Conduct intake interviews and assessments to determine consumer eligibility, needs, and appropriate mental health services.
  • Maintain a caseload of individuals participating in the MH Jail Diversion Program and others involved with the criminal justice system.
  • Coordinate mental health services with courts, correctional facilities, providers, community agencies, and other stakeholders.
  • Attend court hearings and advocate for consumers as appropriate.
  • Assess emergency and immediate-need situations and make appropriate referrals, including facilitating voluntary and involuntary psychiatric admissions when necessary.
  • Coordinate and facilitate access to mental health and case management services to promote continuity of care.
  • Complete required assessments, financial liability determinations, referrals, authorizations, Releases of Information, and other program documentation.
  • Monitor consumer participation and progress through required telephone and face-to-face contacts.
  • Facilitate interagency and ISPT meetings and participate in treatment and discharge planning.
  • Develop and maintain supportive relationships with consumers, families, providers, and community partners.
  • Advocate for consumers and promote resiliency, recovery, and the belief that individuals can overcome barriers to achieving their goals.
  • Maintain accurate and timely case records, reports, and correspondence.
  • Participate in staff meetings, required training, and professional development, including training related to the criminal justice system.
  • Support agency mission, vision, values, and Code of Ethics in all work activities.
  • Complete special projects and other duties as assigned.

Preferred Education & Experience

  • Bachelor's degree with major coursework in one of the following areas: sociology, social welfare, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, education, or a related social science field.
  • Experience working with individuals involved in the criminal justice system or forensic mental health services is a plus.
  • High school diploma and at least 12 semester credit hours in sociology, social welfare, psychology, gerontology, or another social science field, and
  • Two (2) years of experience in public or private human services, including at least one (1) year of direct contact with mental health consumers.

OR

  • High school diploma and five (5) years of mental health direct-care experience, with employment as an ICM staff person prior to April 1, 1989.

Additional Requirements

  • Act 33/34 clearances required.
  • Reliable access to a vehicle is required.
  • Ability to travel throughout the community and attend court hearings, provider meetings, and other required appointments.
  • Ability to work effectively with individuals experiencing mental health challenges and individuals involved with the criminal justice system.

Relocation to Harrisburg, PA, is required before starting.
