Qualifications

• Master's degree in behavioral health field required
• Must possess a valid driver's license with a satisfactory driving record
• Personal vehicle required to provide services in the community
• Must have a professional clinical license or be eligible for a DMH Therapist Certification

Responsibilities

• Behavioral health care experience preferred (but not a must)
• Provides information/referral and resource coordination for individuals and/or collaterals
• Provides services that are directed toward helping the beneficiary maintain his/her highest possible level of independent functioning
• Monitors the individual service plan and ensures team members complete tasks assigned
• Ensures that follow up and follow through occur as needed
• Facilitates pre-cert application process, monitors progress of applications, and educates and involves clients regarding approvals, denials, and appeals
