Job summary

Hope Unity and Freedom Center in Virginia is seeking a Clinical Coordinator to join our multidisciplinary team serving adults with mental health needs. You will oversee admissions, conduct assessments, and develop individualized service plans with clinical supervision support for Case Managers.

Responsibilities include ensuring documentation, managing denials, staff training, program promotion, and compliance with HIPAA and safety training.

Qualifications

• Master's degree in Social Work or a related field.
• Current Virginia LMHP license or licensure eligibility.
• Valid Virginia Driver's License.
• Ability to pass CPR/First Aid and annual TB screening.
• Experience supervising staff or volunteers (preferred).

Responsibilities

• Complete initial and annual comprehensive needs assessments for referred individuals.
• Complete Continued Stay Authorization paperwork for assigned caseload using sound clinical documentation.
• Develop clinically sound individualized person-centered service plans and monitor implementation.
• Manage and respond to pending denials with MCO or agency responsible for authorizations.
• Identify clinical areas that require strengthening and seek training opportunities.
• Provide training to agency staff including new employee orientation and specialized training.
• Promote the program to elicit referrals and maintain census and growth.
• Attend annual mandatory agency trainings (Handle with Care, CPR & First Aid, HIPAA, etc.).
• Perform all duties of case manager as needed.
• Follow CDC mask guidelines during pandemics when required.
• Other duties as assigned.
