Job summary

Macomb County Community Mental Health is seeking a Case Manager for the VDPS program. The role involves delivering direct services to program members and coordinating with families and providers to connect community resources.

The successful candidate will conduct assessments, develop plans, and monitor progress within a 37.5-hour work week. Benefits include health/dental/vision coverage from day one and a starting time of 8:30 a.m.

Qualifications

• Bachelor’s degree in Psychology, Social Work, or related mental health field.
• MDHHS PIHP/CMHSP QIDP qualifications per Medicaid services.
• Bachelor’s in human services satisfies a human services degree requirement.

Responsibilities

• Conducts assessments, service planning, and intensive case management for assigned clients.
• Performs home visits and community-based interventions.
• Maintains MCCMH records, including plans, notes, and reports.
• Provides in-service training to staff and community partners.
• Coordinates with families and aftercare to secure services.
