Qualifications

• Bachelor’s Degree from an accredited college or university in Psychology, Social Work, Special Education, Counseling, or other mental health related field.
• Meet qualifications as specified in the MDHHS PIHP/CMHSP Qualifications per Medicaid Services as a Qualified Intellectual Disability Professional (QIDP).

Responsibilities

• Conducts assessments, service planning, specialized intensive case management for assigned clients.
• Conducts home visits and community-based interventions.
• Maintains MCCMH records including plans, case notes and reports.
• Coordinates with families and aftercare programs to secure services.
• Collaborates with diverse disciplines to ensure timely service provision.
