Job summary

Wellvance in Michigan is seeking a dedicated Case Manager to support individuals with SMI, SED, and/or I/DD by coordinating services that promote independence, community inclusion, and overall well-being.

This role involves advocacy, service linkage, and monitoring to ensure continuity of care and prevent gaps. Preference given to applicants with lived experience in mental illness or developmental disabilities.

Qualifications

• Bachelor’s degree in a human services field.
• Minimum one year of experience in mental health.
• Certification as QMHP, QIDP, and/or CMHP, or Michigan licensure (limited or full).
• Valid Michigan driver’s license, vehicle insurance, and satisfactory driving record.
• Strong organizational, communication, and problem-solving skills.

Responsibilities

• Develop and implement person-centered service plans.
• Link individuals to medical, social, educational, and therapeutic resources.
• Coordinate and monitor services to achieve successful outcomes.
• Provide advocacy and support to reduce barriers to care.
• Participate in emergency on-call rotation.
