Job summary

Macomb County Community Mental Health is seeking a Case Manager - Outpatient North to deliver direct clinical services to adults and families in a community mental health setting.

You will coordinate with health care providers, document treatment plans, and participate in team meetings to ensure patient-centered care. A Bachelor’s degree in a mental health field and QMHP qualifications are required; a Master’s degree is preferred.

Qualifications

• Bachelor's degree in a mental health related field and QMHP qualifications.
• Experience or fieldwork with persons receiving mental health services.
• Valid Michigan driver’s license and ability to meet credentialing requirements.

Responsibilities

• Provide direct clinical services including assessments, crisis intervention, case management, and group activities.
• Maintain accurate clinical documentation and progress notes.
• Coordinate with hospitals, providers, and community agencies to ensure continuity of care.
• Request authorizations, monitor service utilization, and manage re‑authorizations.
• Participate in agency meetings, court hearings, and cross‑disciplinary teams.
• Operate a vehicle to travel to different sites as required.
