The role focuses on assessments, home visits, service planning, and case management to help clients access medical, financial, and social services. Qualified candidates will hold CBHCM certification (or pursue it within specified timelines) and possess a relevant bachelor’s degree.

Qualifications

• Certified Behavioral Health Case Manager (CBHCM) verified through the Florida Certification Board (FCB) per Florida Statute 394.4573(e).
• Must obtain Certified Case Manager – CBHCM within 12 months of hire.
• If provisional CBHCM on hire, must obtain full CBHCM within 12 months.
• Other Bachelor’s degree with three years full-time or equivalent experience with adults experiencing serious mental illness.

Responsibilities

• Assessments: conduct comprehensive intake assessments and document within required timelines.
• Home visits to evaluate safety and well-being; document with compliance.
• Develop individualized service plans with measurable goals and timelines.
• Assist clients in gaining access to financial, employment, medical, and social services.
• Meet service productivity standards and maintain detailed service records.
• Document progress notes within 24 hours and ensure alignment with CSP.
