Job summary

Westchester General Hospital is seeking a qualified professional to assess psycho-social needs of patients and initiate discharge planning. You will counsel patients and families and coordinate resources for post-discharge care.

The role involves group and individual therapy, collaboration with a multidisciplinary team, and ensuring compliance with mental health regulations. Associate's degree in nursing preferred; two years inpatient hospital experience with CM/UR/DCP preferred.

Qualifications

• Assess psycho-social needs of patients and plan discharge.
• Counsel patients and families for post-discharge care.
• Complete psychosocial history and treatment plan per policy.
• Assist with program planning, development and evaluation.

Responsibilities

• Counseling individual patients and family members.
• Make referrals to home health and community agencies.
• Knowledge of Baker Act and mental health laws.
• Coordinate discharge planning and modify plan as needed.
• Assist in program planning and evaluation.
• Perform intake assessments and develop treatment plans.
• Monitor medication-related observations in elderly.
• Consult with physicians on patient plans.
• Implement crisis intervention when needed.
• Conduct individual and group therapy sessions.
• Research community resources and site visits.
• Participate in CQI, safety and infection control.
• Maintain flexible schedule for discharge planning.
• Occasionally may require overtime.
• Perform other duties as required.
