Job summary

Westchester Medical Center Health Network is seeking a Social Work Assistant to provide case management and discharge planning in an inpatient setting. You will be an active member of the Case Management treatment team and participate in daily multidisciplinary meetings on the unit.

The role requires collaboration with the Discharge Planner, upcoming referrals to community agencies, and timely documentation of interactions. Strong communication and ability to handle a high caseload are essential.

Qualifications

• Minimum of 2-3 years' experience in Acute Behavioral Health.
• High School Diploma required; Associate's Degree preferred.
• Excellent communication skills.
• Ability to work in a fast-paced environment and manage a high caseload.
• Ability to collaborate with interdisciplinary staff and external agencies.

Responsibilities

• Work as part of the multidisciplinary team.
• Assist Discharge Planner with post-discharge care and referrals to community agencies.
• Document interactions timely and accurately.
• Maintain professional behavior and positive relations with staff, patients, and community partners.
• Identify problems and communicate them to the Discharge Planner and treatment team.
• Adhere to hospital policies and safe work practices during admissions.
• Develop knowledge of community resources and other concrete services.
• Perform other duties as assigned.
