Zusammenfassung

Community Guidance Center in Indiana is seeking a full-time case manager to provide comprehensive case management services and coordinate care across multiple providers and agencies. You will document care, ensure regulatory compliance, and participate in team meetings and supervision.

The role requires a Bachelor’s degree, FBI/ACT clearances, and at least two years of clinical mental health experience, with strong communication and computer skills.

Qualifikationen

• ACT 33, ACT 34 and FBI clearances required.
• Minimum 2 years of clinical care experience in a mental health setting.

Aufgaben

• Provide case management services and facilitate consumer progress.
• Communicate openly with team members to support continuity of care.
• Liaise with CHIPPS Program, Community Guidance Center, Torrance State Hospital and others to coordinate transfers.
• Maintain complete documentation and ensure compliance with fiscal and regulatory standards.
• Attend departmental meetings, supervision, and maintain quality of care.
