Mental Health - Case Manager (782) (Full-Time)

We are offering a challenging full time Mental Health Case Manager to provide case management to children and adolescents.

Responsibilities

Responsible for performing case management duties with Seriously Emotionally Disturbed youth. Case Management means services that can include assistance to individuals and their family members in accessing needed services that are responsive to the person's individual needs. Case management services consist of identifying potential users of the service; assessing needs and planning services; linking the individual to services and supports; assisting the individual directly to locate, develop, or obtain needed services and resources; coordinating services with other providers; enhancing community integration; making collateral contacts; monitoring service delivery; discharge planning; and advocating for individuals in response to their changing needs.

Required Education and Experience
  • Bachelors degree in human services field or related field .
  • One (1) year position related experience in clinical experience and/or case management services to children and families.
  • Qualified Mental Health Professional (QMHP).
  • Valid Drivers License with a point balance of -6 or higher.
Required Knowledge, Skill and Abilities
  • Knowledge of services and systems available in the community including primary health care, support services, eligibility criteria and intake processes and generic community resources.
  • Fundamental understanding of the nature of serious mental illness, intellectual disability, substance use disorders, or co-occurring disorders depending on the individuals served, including clinical and developmental issues.
  • Competent in various types of assessments, including functional assessment, and their uses in service planning.
  • Comprehensive knowledge of treatment modalities and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning, and service coordination.
  • Proficiency in the types of mental health, developmental, and substance abuse programs available in the locality.
  • Knowledge of the service planning process and major components of a service plan.
  • Understanding of the use of medications in the care or treatment of the population served.
  • Fundamental understanding of all applicable federal and state laws and regulations and local ordinances.
  • Ability to identify and document an individual's need for resources, services, and other supports.
  • Proficiency in using information from assessments, evaluations, observation, and interviews to develop service plans.
  • Ability to identify and document how resources, services, and natural supports such as family can be utilized to promote achievement of an individual's personal habilitative or rehabilitative and life goals.
  • Ability to coordinate the provision of services by diverse public and private providers.
  • Work as team members, maintaining effective inter- and intra-agency working relationships.
  • Work independently performing position duties under general supervision.
  • Ability to engage in and sustain ongoing relationships with individuals receiving services.
  • Excellent communication and organizational skills.
  • Demonstrated competency and proficiency with personal computers
  • Ability to conduct required home visits while drivig agency vehicle to conduct agency business.
  • Ability to make sound clinical judgment.
  • Ability to manage a caseload of 40.

Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.Our Agency maintains a drug-free workplace.

Annual Salary Range

Grade S-C7

$52,576.00 - $59,148.00
