Job summary

Johns Hopkins Hospital is seeking a Psychiatric Care Navigator to coordinate patient discharges and transitions of care within a social work and multidisciplinary team. You will communicate with referral sources, connect patients to community resources, and obtain insurance authorizations as needed.

Qualifications

• Associate degree required, bachelor’s preferred; related experience may substitute.
• Knowledge of mental health care, social work practices and the health care environment.
• Familiarity with utilization review and insurance authorization processes.
• Ability to gather and synthesize information to support care decisions.
• Strong written and verbal communication with patients, families and staff.
• Experience in social work or care coordination in health care or mental health.
• Strong assessment, problem-solving and organizational skills.
