Job summary

Endeavor Health’s Linden Oaks Hospital in Naperville is seeking a Per Diem Mental Health Associate I to provide direct patient care in behavioral health settings. The role includes facilitating therapy groups, ensuring patient safety, and documenting care under the supervision of the interdisciplinary team.

The position requires a Bachelor’s or higher in a Behavioral Health field or related experience, CPI and CPR certifications, and flexibility to work all shifts.

Responsibilities

• Facilitates didactic, educational and unit/program community groups within the assigned treatment programs. Encourages and facilitates patients' participation in therapeutic activities. Supervises patients to ensure their safety and their involvement in treatment.
• Ensures patient safety by providing one-to-one supervision. Unit rounds are conducted, and precautions records completed in real time. Precautions records are accurately labeled.
• Orients new patients to unit, program and milieu by establishing a rapport with patients and families and making necessary introductions. Provides appropriate hand off information when accepting or receiving borders from other units.
• Participates in treatment planning by working with interdisciplinary team to identify patients' problems, needs and strengths, suggests modifications to treatment plan based on observation of patient behavior and provides input into discharge planning.
• Completes patient, program, and department documentation.
• Performs various clerical and administrative assignments to support department operations.
• Monitors and reports patients' progress and communicates information to charge person on assigned shift and family members.
