The Residence at Boyertown is seeking a dedicated Resident Wellness Assistant to provide compassionate daily living support to residents, including personal care, mobility, and hygiene. This role emphasizes dignity, respect, and empathy, applying Memory Care techniques to reduce stress and promote comfort.

You will collaborate with the Wellness Director and care team to implement individualized care plans, maintain a safe environment, and document observations in the EHR.

• Ambulates or assists residents with mobility to activities, dining, and other locations.
• Assist with personal hygiene tasks (teeth, hair, shaving, etc.).
• Lift residents as needed and ensure safe handling.
• Communicate professionally with residents, families, and staff.
• Collaborate with Wellness Director and care team on individualized care plans.
• Maintain a clean, organized medication area and promote safety.
• Provide attentive care respecting mobility, cognition, and sensory needs.
• Build relationships with residents and team while upholding ethics.
