Responsibilities

• Collaborate with nursing, case management, families, and other healthcare professionals.
• Ensure supervision to minimize wandering, falls, agitation, and other dementia-related concerns.
• Provide daily leadership and coaching to staff.
• Ensure regulatory compliance and care planning participation.
• Communicate with residents' families about daily care and quality of life.
