Job summary

Live Well Homecare is seeking an RN Designee to support nursing leadership in an assisted living and memory care community. You will coordinate resident care, guide clinical teams, and ensure safe, person-centered services for residents with dementia.

Ideal candidates have RN licensure and experience in long-term or memory care, with strong clinical judgment and the ability to lead care teams, oversee medication management, and promote quality outcomes while maintaining regulatory compliance.

Qualifications

• Registered Nurse with experience in long-term care and/or memory care preferred.
• Strong clinical judgment and ability to lead and collaborate with care teams.
• Ability to support individualized care plans for residents with dementia.
• Knowledge of medication management practices and compliance requirements.
• Commitment to safe, compassionate, person-centered care.

Responsibilities

• Coordinate and oversee individualized resident care and care planning.
• Support clinical team guidance to promote quality outcomes and safety.
• Help ensure medication services are properly administered and managed.
• Assist with staff training related to clinical practices and person-centered dementia care.
• Support regulatory compliance and adherence to applicable standards.
