Job summary

Avamere at Hermiston is seeking a Medication Tech to administer medications and provide direct care to residents in a Memory Care/Assisted Living setting. This full-time swing shift (2pm-10pm) rotates every 4 days on/2 days off, located at 980 West Highland Avenue, Hermiston, OR 97838.

Responsibilities include passing meds under RN supervision, maintaining accurate records in the EMR, and coordinating with nursing and pharmacy teams.

Qualifications

• Experience as a Caregiver and Medication Tech in memory care or assisted living.

Responsibilities

• Pass medications according to physician orders, delegated by and under the supervision of a licensed nurse and in accordance with facility policy.
• Administer accurately and timely.
• Record medications administered in the resident's record.
• Notify the pharmacy when medications need to be refilled.
• Maintain a clean and organized medication cart and medication room.
• Monitor and report any side effects of medications to the Director of Health Services.
• Complete shift reports with sufficient information for medication orders.
• Follow care plans related to medication administration.
• Assist with medical appointments and communicate with healthcare providers.
• Observe residents for health changes and monitor vital signs.
• Document changes in residents' status in the EMR.
• Complete accident and incident reports promptly.
• Perform delegated nursing services as assigned by the RN.
• Participate in in-service education.
• Report changes in resident conditions promptly.
• Follow written and oral directions.
• Perform duties effectively and professionally.
• Ensure resident safety under supervision.
• Collaborate with residents, staff, physicians, families, and providers.
• Assist new employees with policies and procedures.
• Maintain confidentiality of resident information.
• Perform other duties as assigned.
