Job summary

Avamere at Hermiston is seeking a Medication Tech for a swing shift (2pm-10pm), full-time. You will administer medications and provide direct care to residents in a memory care/assisted living setting.

Responsibilities include recording in EMR, monitoring residents, and coordinating with nursing and health services. Must have CPR/First Aid, be 18+, and maintain confidentiality as part of our care team.

Qualifications

  • Experience as a Caregiver and Medication Tech in Memory Care or Assisted Living.
  • Must have CPR/First Aid Certification during employment.
  • Must be 18 years or older.
  • Experience with electronic medical records documentation.
  • Must obtain Food Handler’s Card.

Responsibilities

  • Pass medications according to physician orders under supervision of a licensed nurse.
  • Administer medications accurately and timely.
  • Record medications in resident records.
  • Notify pharmacy or family when meds need refills.
  • Maintain a clean medication cart/room.
  • Monitor side effects and report to Health Services director.
  • Follow care plans based on each resident’s needs.
  • Assist with medical appointments and communicate with providers.
  • Observe residents, report health changes, monitor vital signs.
  • Document changes in EMR and complete incident reports.
  • Perform delegated nursing duties as assigned by RN.
  • Participate in in-service education and team communication.

Medication Tech

Type: Full-Time

Shift: Swing shift (2pm-10pm), 4 days on/2 days off/rotating schedule

Location: Avamere at Hermiston- 980 West Highland Avenue, Hermiston, OR 97838

The primary role of the Medication Tech is to administer medications and provide direct care to residents.

Essential Duties and Job Responsibilities
  • Pass medications according to physician orders, delegated by and under the supervision of a licensed nurse and in accordance with facility policy.
  • Administer accurately and timely.
  • Record medications administered in the resident’s record.
  • Notify the pharmacy (or family when applicable) when medications need to be refilled.
  • Maintain a clean and organized medication cart and medication room.
  • Monitor and report any/all side effects of medications to the Director of Health Services.
  • Obtain/provide shift report. Record sufficient information related to medication administration to implement accurate changes to medication orders.
  • Follow comprehensive care plans related to medication administration based on each resident’s needs and their comprehensive assessment as assigned.
  • Assisting with medical appointments, communicating with healthcare providers, and advocating for the residents’ needs
  • Observing residents for changes in health, reporting concerns to medical professionals, and monitoring vital signs.
  • Document information pertaining to changes in residents’ status, personal needs, and communications with outside providers in the electronic medical record.
  • Complete accident and incident reports in a timely manner and according to facility policy
  • Perform delegated nursing services as assigned by the RN.
  • Participate in in-service education programs as assigned.
  • Promptly report changes in resident conditions to the Director of Health Services.
  • Follow written and oral directions.
  • Perform all duties assigned in an effective, cooperative, timely and professional manner.
  • Be responsible for the safety of residents under their supervision.
  • Work cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies, and ancillary service providers.
  • Assist new employees in following established facility policies and procedures.
  • Maintain confidentiality of resident and facility information.
  • Perform other duties as assigned by the manager or supervisor.
  • Experience as a Caregiver and Medication Tech in a Memory Care Unit and or Assisted Living Facility.
  • Must have the ability to obtain and maintain active CPR/First Aid Certification during employment.
  • Must have the ability to obtain a Food Handler’s Card.
  • Experience in electronic medical records documentation.
  • Must be 18 years or older.
